We provide in-home medical care to individual patients in their homes or locations of residence in the following counties in North Carolina: 

 Durham County

Wake County 


In order for us to provide this service, patients will be responsible for a $125 service fee (not covered by insurance) for on-site clinician services.

Medical Support

 Our team will support you in building a healthier you. No matter what your health needs are, having a team that is patient centered will keep you on the path to meeting and achieving your goals. We work together to connect you with the services you need. 


We provide on-site primary care services in multiple communities in North Carolina on a weekly and/or monthly basis based on patient needs. In addition to primary care services, we coordinate with 3rd party vendors to also enhance patient care and provide on-site imaging services and laboratory services to support our comprehensive care model.

Insurance Coverage

We are in-network with Medicare and Medicaid and accept all supplemental insurances!

 Medicare and Medicaid provides payment for medical services in a patient's home and/or location of residence.  

All visits will be billed to primary insurance provided as well as to any secondary or supplemental insurance policies.  Check our our current policies we are in-network with by clicking link below.


Supplemental Insurance Policies:   Medicare Supplement (Medigap) plans also generally don't have networks. Medicare Supplement plans may cover some out of pocket costs that Original Medicare doesn't cover, such as deductibles, copayments, and coinsurance. We submit our visits to all primary insurance policies and then it is automatically submitted to any supplemental insurance policies provided.  Check out the link below to learn more about supplemental insurance policies and please contact the customer service number on the back of your supplemental policy if you have questions regarding in-network requirements for providers.

Urgent Care Services: In order to best support our patients with urgent care needs, we offer clinician availability on non-scheduled facility days for in-person medical care.  The $125 trip charge associated with our in-person urgent visit is not covered by Medicare and Medicaid and would be out of pocket.

In-Home Visits: We also provide medical care to patients that reside in their home.  We do require a $125 trip charge that is out of pocket with our in-person home visits that is not covered by Medicare or Medicaid. Laboratory & Imaging Services: We contract with 3rd party vendors (Quest Laboratories, Dispatch Imaging, National Mobile X-Ray ) that are in-network with Medicare, Medicaid, and many commercial insurances to provide laboratory and imaging services on-site and all services are billed to patient's insurance.

SHAE Medical NOW ParTnering with RevUp between visits

We are excited to announce that our practice is now offering a new, Medicare-covered* benefit!

The program includes the following benefits:

  • A personalized health coach --- provided from a dedicated RevUp Health Care Team, led by a Registered Nurse.
  • Home devices that automatically send vitals to your Health Care Team--Devices can include monitors for blood pressure, heart rate, blood sugar, weight, or oxygen levels and are selected for you by your SHAE Medical provider.
  • The devices come with a Hub that is cellular connected, so internet or WiFi is not needed!
  • The system also includes an emergency response system that will connect you to help 24/7 at the touch of a button.
  • Ability to ask health questions anytime using a computer, smartphone, or over the phone
  • Privately track important health information that is shared only with your RevUp Health Care Team and your SHAE Medical Provider
  • Your SHAE Medical Provider will review regular updates on your on-going care to help you stay connected between office visits
  • Caregiver Program: If you would like some help with RevUp, with your permission, you can allow a trusted family member or caregiver to record your information on your behalf. Your caregiver will have full access to your RevUp account and can communicate with the RevUp Health Care Team


*Like all Medicare Part B services, a co-insurance payment is required. If you have a Medi-Gap-type supplemental medical policy, you may not be responsible for any payment. Patients who have not met their deductible for the year may be responsible for meeting their yearly deductible first, if they do not have secondary insurance or if their secondary insurance does not cover deductibles.
